This Statement should describe the care that you provide for your patients and needs to contain enough information to allow the CQC to decide whether you are meeting the essential standards of quality and safety.

It should be reviewed at regular intervals and updated, when necessary, to include any significant changes to your service and any change must be to notified to the CQC.

It is important that practices have one, as the CQC may ask to see it at any stage - for example during a practice inspection.

The form asks you to indicate which regulated activities you undertake and those for primary dental care providers are:

Treatment of disease, disorder and injury
Surgical procedures
Diagnostic procedures.

The example Statement included here uses the CQC’s templates and ensures that you provide the information that is required by law. Our guidance appears in the boxes - simply delete these notes and insert your own text to generate your own practice-specific Statement.

You do not need to use the template and can, if you prefer, develop something that suits your needs; however, these templates will help you identify the information that is required.

There are four separate sections:

Part 1. The provider’s name, legal status, address and other contact details, including the names of all the partners (if you are in a partnership)

Part 2. This deals with your aims and objectives in providing the regulated activities and we have provided some possible answers for you to rephrase into your own words

Part 3. This relates to the practice location, the people who use the service, the service type and the regulated activities provided

Part 4. This concerns the registered manager details. You must have a registered manager if you are a partnership or Limited Company (and that person can be one of the providers).
